If you are a new or old patient , please use the following checklist to prepare for your appointment. 

• Your Current Insurance Card

• Your Current Driver's License

• Your Co-pay or Co-insurance  (cash, check or credit card, (we do not take American Express)).

• All the medication you take including:  Prescriptions, Over the Counter, Herbal Products, and Vitamins.

• A list of questions you want to ask.
